Neuro-IOM-16-V is a cutting-edge modular multi-functional platform solution for intraoperative neurophysiological monitoring (IONM).  Its application allows the surgical team to be informed about possible damage of nerve tissues resulting from surgeon actions and, thus, to prevent neurological deficit and improve patient outcomes.

 

Neuro-IOM-16-V is the essential configuration for IOM in veterinary surgery that allows performing intraoperative neurophysiological monitoring using 16 channels.  The system also includes electrical stimulator channels and transcranial electrical stimulator channels to control peripheral and motor pathways.

 

In many countries, intraoperative neurophysiological monitoring is required in various types of surgery, i.e., spine surgery, brain surgery, brainstem surgery, etc.  In cases of spine surgery (for example, scoliosis), the tactics is directly determined during IONM.

 

Fundamental IONM with expert-class functionality

 

The Neuro-IOM-16-V system is equipped with the acquisition and stimulator pods to attach recording and stimulating electrodes with 5-meter cables.  It allows arranging the system at a distance from the operating table to ensure free movement for the surgical team.  

 

The Neuro-IOM-16-V system performs intraoperative neurophysiological monitoring of all major modalities, including Myogenic Evoked Potentials (MEP) acquisition using the electrical stimulator with stimulus current of up to 200 mA.

 

The set of specially-designed electrodes and the software are also included to the Standard Set.

 

Neuro-IOM-16-V key features:

 

· 16 amplifier channels.

 

· 2 dedicated channels for routine Electromyography (EMG), Nerve Conduction Studies (NCS), and Evoked Potentials (EP).

 

· 4 channels for electrical stimulation.  In most cases, 4 electrical stimulator channels are sufficient for Somatosensory Evoked Potentials (SEP) and Myogenic Evoked Potentials (MEP) acquisition.  For example, two channels are used for the stimulation of lower limbs during the acquisition of SEP responses, one channel is used for Transcranial Electric Stimulation (TES), and the other one is left as a stand-by channel.

 

· Direct nerve stimulation (DNS) channel.

 

· Auditory, visual and pattern stimulator channels.

 

· Electrical Stimulator (ES) detector channels.

Two-in-one:  IONM device and 2-channel EMG/EP system

 

Neuro-IOM-16-V can be used as an EMG/EP system.  For this application, acquisition channels and all necessary stimulators are built in.

 

Motor, somatosensory, auditory and visual evoked potentials, EMG, direct nerve stimulation, EEG, ECoG – more than 10 modalities for intraoperative neurophysiological monitoring.

 

When the Neuro-IOM-16-V system is not in the operating room, it can be used as a common EMG system. Not just an EMG system, but the one produced by Neurosoft!

 

Neuro-IOM-16/V can be used as the EMG/EP system, the acquisition channels and all necessary stimulators are built in.

 

Pre-set testing modalities

 

The Neuro-IOM-16/V system is delivered with the pre-set templates for the following modalities:

 

·  Direct nerve stimulation – to monitor peripheral nerve integrity.

·  Free-run Electromyography (EMG).

·  Somatosensory Evoked Potentials (SSEP) – to control sensory neural pathways.

·  Motor Evoked Potentials (MEP) – to control motor neural pathways.

·  Auditory Evoked Potentials (AEP) – to control auditory nervous pathways.

·  Visual Evoked Potentials (VEP) – to control visual nervous pathways.

·  Electroencephalography (EEG) – to control the brain activity.

·  Electrocorticography (ECoG) – to control cortical function.

·  Direct cortical stimulation – to monitor peripheral nerve integrity.

·  Train-of-four stimulation (TOF) – to assess the depth of neuromuscular blockade.

 

Using these modalities, the surgical team can control motor and sensory pathways, monitor peripheral nerve integrity, assess the brain activity and map its functional areas, etc.

 

Neuro-IOM-16-V main system

 

The Neuro-IOM-16-V system is equipped with the acquisition and stimulator pods to attach recording and stimulating electrodes with 5 m cables.  It allows arranging the system at a distance from the operating table to ensure free movement for the surgical team.  The set of specially-designed electrodes and the software are also included to the Standard Set.

 

Variety of stimulators

 

The system can be operated with a wide variety of stimulators – transcranial electrical stimulator (up to 1000 V), 4-channel electrical stimulator, low current stimulator, auditory, and visual stimulators.

 

Robust stimulators ensure high-quality responses during acquisition of brain evoked potentials and EMG exams.

 

Transcranial electrical stimulator deserves the special attention.  It is intended for stimulation of motor cortex to record motor evoked potentials from muscles.  The stimulator ensures single pulse and train stimulation, as well as double train and train + pulse stimulation.

 

Flexible configurations to satisfy any monitoring needs

 

The Neuro-IOM.NET software provides a set of default templates for various types of surgeries.  Each template includes the settings of modalities for a particular type of surgery, test window layouts, acquisition and stimulation parameters, etc.  The system has a large number of channels and stimulators with separate adjustment.  Preset templates simplify and speeds up the system setup for surgery.

 

You can always change and save the settings of any surgery template.  It is a unique option for this kind of systems. There is no IONM system in the world that is so flexible!

 

High acquisition quality:  Sampling rate up to 50 kHz in IOM channels and 100 kHz in EMG channels

 

High sampling rate and analog-to-digital conversion (ADC) capability allow obtaining recorded traces of exceptional quality under any circumstances:

· Perform acquisition without special grounding, when computer is powered from a common three-wire outlet.

· Obtain sensory responses even after the first stimulus (without averaging).

 

Neuro-IOM-16-V channels are optimized to record needle EMG, with immunity to noise considerably increased.  It makes needle-EMG study faster and improves the quality of needle-EMG acquisition.

 

Video-recording during surgery

 

Video from up to three video cameras and from operating microscope can be displayed on the screen and recoded in the file.  The recorded video is synchronized with other monitored data.

 

Operation in unshielded Operating Room environments

 

Neuro-IOM-16-V is thoroughly shielded from electro-magnetic interferences (EMI) and can be used in such harsh environments as Operating Rooms (OR).

 

Advanced analysis tools

 

The software provides the clinician or researcher with the necessary tools to analyze recorded responses.

 

Intelligent software with Veterinary-specific algorithm

 

The supplied software features a veterinary-specific algorithm.  It ensures EEG recording, calculation of all major parameters, analysis, and report generation.  

Exceptionally high-quality low-noise physiological preamplifiers, combined with high-efficiency wide-dynamic-range analog-to-digital conversion, digital filtering, and digital signal-processing algorithms, were specifically designed for Neuro-IOM-16-V based on extensive research and development.  They ensure excellent noise suppression, clear signal recording, and reliable, reproducible calculation of recorded parameters.

This allows not only better diagnostics, but also reliable monitoring for any changes in follow-up assessments over prolonged periods of time.

 

Intra-operative EEG monitoring

 

Designed to work in the electro-magnetically harsh Operating Room (OR) environment, Neuro-IOM-16-V can be used as a reliable monitor during surgery and post-operative recovery.

 

IONM reports

 

Reports – with recorded traces, numeric data, and comments – are generated on-screen, can be saved, printed out, and shared.

 

Database and data export

 

Measurement data and interpretation, as well as exam reports, are saved in a well-organized software database.  The data and reports can be printed out either at the time of the assessment, or at a later time, and shared with a referring doctor or researcher.

Recorded parameters can be exported to Excel spreadsheets and other statistical software for further data analysis making it an ideal tool for research projects.

 

Software compatibility

 

Neurosoft Software enables highly efficient operation – saving both capital and operating costs – through a number of compatibility features:

 

Multiple Neurosoft Software:  Neurosoft Software for various Neurosoft instruments can be installed on the same computer – with no conflict between them.  Moreover, all software can share a single database – for greater convenience and work-flow efficiency.

 

 

Multiple computers:  Although only one computer can be used for diagnostic test data recording, each Neurosoft Software License allows using software on an unlimited number of computers.  Thus, the data can be recorded on one computer, and then, the data can be shared, analyzed, reviewed on other computers – with no additional Software Licenses.

 

Other applications:  Neurosoft Software can operate on a computer with Microsoft Office applications – Word, Excel, Outlook, PowerPoint, Access, Project, as well as Adobe and accounting software.  This is particularly efficient in smaller clinics.

 

Internet:  Computers with Neurosoft Software installed can be connected to the Internet.  In an unlikely and rare event that a Microsoft Operating System update affects a Neurosoft software component, it can be restored by Neurosoft software re-installation, with no loss of data. 

 

Information system integration

 

Most clinics, networks and research institutions are equipped with information systems to store and process their data.  HL7 (Health Level 7) is the most commonly used interface engine for exchange, integration, sharing and retrieval of electronic data.

 

HL7-compatibility allows integrating Neuro-IOM-16-V into the information system of a veterinary clinic, clinical network, or research institution.

 

Disinfection

 

Electronic Unit – soft cloth gently wrung in phenols (Bacillotex® etc.), or 70% alcohol, or 0.5% chlorohexidine.

 

If dangerous virus contamination is suspected – aldehydes (Cidex® etc.), or chlorinates (Diversol BX® etc.).

Electrical Safety and Electromagnetic Compatibility

 

Electromagnetic compatibility (EMC) is ensured in compliance with the requirements of EN 60601-1-2-2015 standard.

 

The digital system is intended for operation in electromagnetic environment, which special features are specified in Annex 1.

 

The mobile radio-frequency equipment can impact the system.

 

The use of accessories not listed in Table 3 and Table 4 of this technical manual can cause the increase of electromagnetic emissions or decrease of the electromagnetic immunity.

 

As for safety, the digital system complies with the requirements of EN 60601- 1:2006/A1:2013 and EN 60601-2-40:2017 standards. The electronic unit is supplied by AC mains 220 V, 50 Hz, it has double isolation and CF type work parts according to EN 60601-1:2006/A1:2013.  All computer equipment should comply with EN 62368- 1:2014.

 

The current-impedance dependence for electrical stimulator in the range of load impedances is specified in Annex 2.

Minimum requirements to PC:

 

· For the software based on Microsoft .NET platform dual core processor with ≥ 2.6 GHz clock rate and random-access memory (RAM) of ≥ 2 GB is required (4 GB is recommended).  For other software, the clock rate is desired to be ≥ 1200 MHz and RAM ≥ 512 MB.

 

· Either Windows 8 or Windows 10 operating system is required.  Both 32-bit and 64-bit editions are supported.

 

· A ground wire must be available as the third wire of the power outlet.  It is not allowed to connect the computers using adaptors that insulate the third wire.

Electronic Unit limited warranty term – 2 years.

 

During the warranty term, repair is free, and if repair is impossible, Electronic Unit is replaced.  Warranty does not cover mechanical damage and unauthorized opening of Electronic Unit, and defects caused by misuse, and any product that has been altered or modified in any way.

 

Warranty repair is professionally performed at the Neurosoft Service Center in Ivanovo, RF.  Transportation costs to the Service Center are covered by the Customer, return transportation is covered by Neurosoft.

 

Customer pays return shipping charges.  We are responsible for the return shipping charges of replaced / repaired products.  Products found to be non-defective will be returned at customer's expense.
